What is ear wax and why does it build up?

Ear wax (cerumen) is a natural substance produced by glands in your ear canal. It plays an important protective role — trapping dust, bacteria, and other particles before they can reach the eardrum.

For most people, the ear is self-cleaning. Old wax gradually moves to the outer ear and falls away naturally. However, for many people this process breaks down, leading to a build-up that can cause discomfort and hearing difficulties.

 

Ear wax blockages are one of the most common ear problems seen by healthcare professionals, affecting around 1 in 10 children, 1 in 20 adults, and 1 in 3 older people.

Who is most at risk?

Certain factors make ear wax build-up more likely:

Hearing aid users — devices can prevent wax from migrating outward naturally.

Frequent use of earbuds or headphones — can push wax further into the canal.

Cotton buds — contrary to popular belief, these push wax deeper rather than removing it.

Age — wax becomes drier and harder to shift as we get older.

                     Never attempt to remove ear wax yourself using cotton buds, ear candles, or sharp objects. These can damage the delicate ear canal or eardrum. Always seek professional removal.

Does ear wax removal hurt?

No — when performed by a trained clinician, ear wax removal is generally comfortable and well-tolerated. Microsuction may produce some noise and a mild sensation. Irrigation uses warm water at body temperature to minimise discomfort. Most patients are pleasantly surprised by how quick and easy the procedure is.

Do I need to prepare my ears before the appointment?

We recommend using olive oil or sodium bicarbonate ear drops two to three times daily for three to five days before your appointment if possible. This softens the wax and makes removal easier and more comfortable. If you can't do this in advance, don't worry — we can still assess and often treat your ears in the same appointment.

How long does the appointment take?

A standard ear wax removal appointment takes around 20 to 30 minutes. This includes the initial ear examination, the removal procedure, and time for aftercare advice. For very impacted wax, it may take slightly longer.

Is microsuction safe?

Yes — microsuction is considered the gold standard method for ear wax removal and is used by ENT (ear, nose and throat) consultants in hospitals. It involves no water and allows the clinician to view the ear canal throughout, making it one of the safest and most effective options available.

Can I have ear wax removal if I have a perforated eardrum?

Yes — microsuction is suitable for patients with a perforated eardrum or grommets, as it uses no water. Irrigation, however, is not suitable in these cases. Please inform us of any known eardrum issues when booking so we can prepare appropriately.

Will it clear my hearing immediately?

Most patients notice an immediate and significant improvement in their hearing following wax removal. Some patients may experience mild, temporary fluctuations in the hours after treatment as the ear canal adjusts — this is normal and settles quickly.

How often will I need ear wax removal?

This varies between individuals. Some people only ever need treatment once, while others find they build up wax regularly and benefit from a routine appointment every six to twelve months. Hearing aid users often find more frequent visits helpful. We'll advise you based on what we find.

Do I need a GP referral?
